Designed for pets with a confirmed diagnosis who are clinically stable and appropriate for nutrition-led intervention.

Who This Package Is For

This service is appropriate for pets with:

  • One primary medical diagnosis

  • Stable or mildly abnormal lab values

  • No acute or advanced complications

  • Ongoing veterinary care already established

If your pet has multiple diagnoses or rapidly changing labs, the Advanced Medical Nutrition package is recommended.

IMPORTANT: In order to make sure I create the best diet possible for your pet please email me (wellsnutrition@gmail.com) any veterinarian made health diagnosis, blood work, urinalysis, stool samples or additional testing you have had received.

✔ 3 emailed check in’s (with diet adjustments if needed) over a 3 month period leading up to follow up testing recommended by your veterinarian to make sure the plan is working!

Clinical Nutrition Consultation

Once your intake forms and medical records are received, you’ll be contacted to schedule your extended consultation.

During your appointment, we will review:

  • Veterinary diagnoses and current treatment plans

  • Bloodwork, urinalysis, and relevant diagnostics as outlined by your vet

  • Diet history and nutrient adequacy

  • Medication–nutrient interactions

  • Lifestyle and environmental factors that may impact disease progression

The focus is to create a nutrition strategy that supports the diagnosed condition while maintaining whole-body balance.

Your Disease-Specific Nutrition Guide

Nutrition plays a critical role in disease management — but only when tailored correctly.

Your personalized plan may include:

  • Adjustments to protein, phosphorus, fat, fiber, or micronutrients as appropriate

  • A feeding plan aligned with current lab values and clinical goals

  • Supplement recommendations chosen for safety and relevance

  • Practical guidance to ensure the plan is realistic and sustainable

Written plans are delivered by email within 3–5 business days.
